Why InovCares exists

Maternal risk is clinical — and structural.

The CDC estimates more than 80% of pregnancy-related deaths are preventable. Black women face two to three times the risk of white women. And the most common non-medical reason moms drop out of prenatal care is simply not being able to get there.

Clinical

Real-time blood-pressure and glucose alerts, linked to OB care.

Cultural

A bilingual experience with culturally concordant clinicians and doulas.

Structural

Rides, groceries, prescription delivery and prenatal vitamins.

Financial

Clinical and non-clinical services reimbursed by the health plan.

One platform

Three front doors, one shared record.

Her health plan, her health center, her OB/GYN, her nurse midwife, her doula and her family all see the same record — and so does she.

Trusted by two Blue Cross plans and four partner sites across Texas and Illinois.

For moms

Pregnancy, postpartum and baby’s first year — with providers and doulas who reflect your community, a medical kit at home, and rides, groceries and prescriptions when you need them.

Our care model

The mother at the center of one coordinated system.

Our model connects the clinical team, family and community support, plans and health-center infrastructure around her. It does not replace existing partners — it runs alongside and unifies them. And it is measured, not asserted every figure on this site traces to a named cohort, published paper, or a partner’s own survey.

A member journey

Chinenye’s pregnancy, step by step.

A history of hypertension, a second-trimester scan showing her son was not growing as expected, and history of a difficult first pregnancy— in her words, and her clinic’s.

Chinenye with her son Xavier

InovCares member · HHM Health, Dallas

Chinenye and Xavier

“This pregnancy was a little rough for a few months, but my first pregnancy was even harder.”

Her story was told by Blue Cross and Blue Shield’s parent company, HCSC. Every step below is from that account.

  1. At a routine prenatal visit

She enrolls where she already goes

Chinenye signed up during a standard prenatal visit at HHM Health in Dallas — no new clinic, no new provider to meet.

History of hypertensionGrowth concern at 20 weeks
  2. Shipped to her door

    She leaves with the kit

A blood-pressure cuff and monitor arrived at home, with a glucose monitor where risk calls for one, paired to the app she already had.

    BP cuff and monitorGlucose monitor
  3. As each reading posted

    Her provider was alerted automatically

Elevated readings went to her care team as they were taken, which is what made quick intervention possible.

“The app gave me peace of mind to know that providers could see my blood pressure and other numbers.”

    Automatic alert
  4. Closer watch

    A specialist stepped in

She was referred to maternal-fetal medicine for closer monitoring and blood-pressure management, and moved to weekly visits.

  5. What stood between her and care

Rides, prescriptions, and a doula who stayed

No-cost rides to appointments, prescriptions she could order, and behavioral health she could reach — after a first pregnancy that brought preeclampsia, depression and panic attacks. Yesenia, her bilingual InovCares doula, met with her regularly before the birth and again after it.

“They were like family, always asking if there was anything I needed.”

  6. 37 weeks

    Xavier arrived safely

Xavier’s growth improved over the weeks that followed. He was born at 37 weeks, and Chinenye is at home in Dallas with two healthy sons.

Why it holds together

Readings, alerts, visits, rides and outcomes sit on one longitudinal record, so none of this depends on a fax, a phone tree or a second login — and every step of it can be shown to the plan afterward. Read Chinenye’s story in the HCSC newsroom.

The platform

Six services. One member record.

Member app, clinician app and population dashboard share the same data — nothing is re-keyed.

  • Adama Assist™

A pregnancy- and postpartum-aware assistant covering 100+ maternal health topics in plain language. Every answer is cited to named physicians, and a clinician reviews it before anything is acted on.

  • Monitoring & health reports

Blood-pressure, glucose and self-ultrasound readings taken in the app, with a health assessment that screens mental, metabolic and social needs — food insecurity, transport, housing — and reports back to the care team.

  • Culturally sensitive telehealth

Scheduling, messaging and video visits with providers who reflect the community — available in 68 languages.

  • Prescriptions & groceries

Prescription orders and delivery, plus $50 a month of groceries and prenatal vitamins to the door — with her own readings and health assessment steering what the catalog recommends.

  • Ride booking to clinics

In-app rides to prenatal visits and delivery hospitals, coordinated across Texas and Illinois.

  • My Tribes & rewards

Health communities with doulas, yoga and education — plus personal health tracking and wellness rewards. Every member is enrolled in a tribe, and across the two New Horizon patient surveys, 97% and 100% said they like being part of theirs.

Why the in-between matters

The in-between steps are where crises are prevented.

A care plan names the visits. What decides the outcome is everything between them — whether a rising reading is seen, whether a ride shows up, whether anyone follows up after birth.

Maternal mental health

Mental health support is part of the care

Depression screening at every postpartum visit — 100% EPDS at Chicago Family Health Center. Behavioral health, counseling and doula support are requested in the app and documented when they are delivered, with a peer community in My Tribes between visits.

The Cocooned Moms Project with the University of Miami

An NIH-funded study led by Dr. Judite Blanc. The InovCares app was deployed at the University of Miami in 2025 and has enrolled participants. The study tests whether therapy delivered through the app lowers stress, depression and blood pressure among pregnant Black women.
